EmpowerED Pathways, LLC builds the coordination that's missing when a student returns to school from psychiatric hospitalization, residential treatment, foster placement, or an alternative setting. We provide pre-discharge planning across facilities, schools, caseworkers, and caregivers; structured reentry protocols districts can implement and measure; and cross-training for school counselors, social workers, foster care caseworkers, and CASAs.

We help students returning from restrictive placements — especially youth involved in foster care — and the systems responsible for them: schools receiving students without a plan, caseworkers absorbing preventable disruptions, and agencies paying repeatedly for placements that fail at reentry.
